What Is Revisional Bariatric Surgery?
Revisional bariatric surgery is a surgical procedure performed for patients who previously underwent bariatric procedure but could not achieve sufficient weight loss or developed postoperative complications. This procedure is conducted on the purpose of repairing or improving outcomes of the previous bariatric surgery. The revisional bariatric surgery can be performed in different methods, and the selected method can be changed based on individual condition and preoperative evaluation of patients. Main aim of the procedure is to revise previously performed surgical operation in order to encourage weight loss, repair complications or eliminate health problems.
Which Methods Are Practiced in Revisional Bariatric Surgery?
- Gastric Bypass Revision:
The revisional methods practiced in patients with history of gastric bypass surgery are as follows:
- Distal Gastric Bypass: The connection between the stomach and small intestine is changed in this method. A new connection is created in lower zone, aiming less food absorption.
- Omega Loop Gastric Bypass: A new connection is created between the stomach and small intestine in this method. This new connection can increase the efficacy of the gastric bypass.
- Gastric Sleeve Revision:
The revisional methods practiced in patients with history of gastric bypass surgery are as follows:
- Revision of Gastric Sleeve: Volume and shape of the previously performed gastric sleeve are changed in this method. This method is performed to improve results of previous surgery when weight loss is intended to be increased or weight loss stops.
- Duodenal Switch: In this surgery, changes are made on the small intestine in addition to gastric sleeve. Food absorption reduces and weight loss is encouraged.
- Gastric Band Removal and Revision:
The revisional methods practiced in patients with history of laparoscopic adjustable gastric band surgery are as follows:
- Band Removal and Gastric Sleeve: The adjustable gastric band is removed and formed as gastric sleeve.
- Band Removal and Gastric Bypass: The adjustable gastric band is removed and gastric bypass procedure is performed.
- Gastric Bypass Revision with Band:
In patients who underwent gastric band surgery before, a gastric band can be used while performing gastric bypass procedure.
- Biliopancreatic Bypass Revision:
This method is performed for revision in patients with history of bypass surgery. The connection between the stomach and small intestine is either changed or revised.
- Distal Ileal Interposition: In this method, a portion of the small intestine is removed and a reconnection is created in patients with history of biliopancreatic bypass surgery. In this way, weight loss can be increased or results of previous surgery can be improved when weight loss stops.
- Mini Gastric Bypass Revision:
- Mini Gastric Bypass Expansion: The connection between the stomach and small intestine is expanded in patients who underwent mini gastric bypass surgery before. This method can be conducted to increase weight loss.
- Adjustable Gastric Band Revision:
The band can be adjusted and repaired again in patients with history of laparoscopic adjustable gastric band surgery. This method can be practiced to increase weight loss or restore efficacy of the band.
